As your Health and Wellness Coach, I will help you explore your values and strengths, support you as an ally, and offer accountability so that you may reach your goals.

…will assist you in identifying creative solutions for you to move toward overall well-being,

…will listen to you attentively and without judgment or my own agenda,

…am committed to being honest and forthright with my feedback,

…will promote realistic expectations throughout our discussions with regards to goal setting,

…will ask questions when needed and encourage you to arrive at your own answers,

…will assist you in ways to work through barriers that may arise,

…will ask your permission before providing advice or direction, and

…will suggest appropriate referrals for topics that arise outside of my scope of practice.

As my client, you

…acknowledge that you are fully responsible for your own progress through your choices and decisions,

…will participate wholeheartedly with me, your coach,

…agree to openly and honestly share any personal information related to your health and wellness,

…understand that information discussed will be held as confidential, unless there is a concern about harm to yourself or others,

…understand you may have setbacks in your behaviors and that these are a normal part of behavior change,

…may ask for resources, support and feedback from me, your coach,

…can choose to discontinue coaching at any time during this process,

…will commit to being on time to our sessions, and

…recognize that coaching is not a substitute for counseling.
